MHA IRA Mitigation Solutions

What does the Inflation Reduction Act (IRA) mean for your pharmacy?

As of January 1, 2026, new provisions under the Inflation Reduction Act (IRA) change how pharmacies purchase and are reimbursed for certain Maximum Fair Price (MFP) drugs included in the Drug Price Negotiation Program (DPNP).

Pharmacies that are not enrolled in the Medicare Transaction Facilitator Data Module (MTF DM) risk PBM claim rejections and/or delayed manufacturer refunds on MFP drugs.

Guiding pharmacies through a new era of pharmacy reimbursement policy

The IRA requirements introduce new complexities for long-term care (LTC) pharmacies. Our 5-Point IRA Solution is built to help LTC pharmacies adapt to the updated IRA provisions with confidence. Our solution combines protection, visibility, technology, advocacy, and insight to help ensure our members are ready to navigate the road ahead.

MHA’s Comprehensive 5-Point IRA Solution

1. Reimbursement Protection through the LTC Reimbursement Network

MHA’s decades long experience and partnerships enable our industry-competitive, multi-year PBM contracts and designed for rate protection, consistent reimbursement, and flexibility. With LTC pharmacy members at the heart of our collaborations with PBMs, we strive to ensure each agreement helps pharmacies remain stable amid IRA-driven reimbursement changes. We do this through specific negotiated terms regarding MFP drugs including:

  • Dispensing fees
  • Reimbursement payment terms
  • Innovative contracting that provide flexibility for certain market conditions
  • Protection against WAC drops

2. GPO and Purchasing

With 35 years of group purchasing experience, MHA is the largest post-acute GPO in the nation. We pride ourselves in the relationships we’ve built with our life sciences partners. In the midst of IRA changes, we’re pleased to continue to offer the following on contracted MFP drugs:

  • Maintaining manufacturer discounts and rebates
  • Clinical resources, data, and insights on therapeutic alternatives to help mitigate impact

3. Visibility and Control

Through Net-Rx, an MHA solution, pharmacies gain visibility into PBM reimbursements and manufacturer refunds on MFP drugs within a single platform. The RecRx platform includes pharmaceutical manufacturers as a payer type, allowing pharmacies to reconcile MFP refund payments in the same way they manage PBM reimbursements. This 360-degree view helps pharmacies identify missing or short-paid refunds, track expected payments, and strengthen cash-flow management with accuracy and clarity.

4. Data and Insights

MHA has developed a proprietary IRA Dashboard that calculates the pharmacy’s dispensing data to give pharmacies an estimate of the financial impact of IRA/MFP drugs on their business. The innovative tool updates as new factors emerge — from contract updates to manufacturer WAC adjustments — providing real-time insight into the pharmacy’s performance.

The IRA Dashboard adapts as the market evolves, offering clarity and informed guidance to help pharmacies plan ahead with confidence.

5. Advocacy

MHA’s legislative team ensures that pharmacy voices are heard in the discussions that shape the future of healthcare through the following integrated activities:

  • Collaborating with industry associations such as NCPA, ASCP, and SCPC, we represent our members’ interests at the highest levels of policymaking
  • Partner with industry experts and engage legislators to provide timely updates and informative webinar sessions that help pharmacies prepare for regulatory change
  • Leveraging lessons to be learned throughout the IRA implementation in 2026 to advocate for LTC pharmacies into 2027

MHA’s comprehensive IRA Solutions equip pharmacies with the tools, data, and support needed to help navigate IRA changes as smoothly as possible.

From negotiating sustainable PBM contracts to delivering real-time reimbursement visibility and advocating for clarity in policy, our approach helps give pharmacies the stability and foresight to manage both today’s requirements and tomorrow’s challenges.

As the industry continues to evolve, MHA remains a trusted partner to help pharmacies understand, adapt, and thrive in a new era of reimbursement.
